On the initial home visit, a member of the organization will discuss our services in more detail, and your right to chose between these available services, in order to assure that you have a complete understanding of the care you will receive.

If at any time your eligibility for services changes or you have been denied eligibility of services, you will be notified in writing. You have the right to appeal any adverse actions concerning eligibility.
